Adaptação intercultural para a versão espanhola do questionário “Structured HIstory of Medication Use” para conciliação medicamentosa na admissão
CONTEXTO E OBJETIVO: O questionário “Structured HIstory of Medication use” (SHIM) é uma ferramenta desenvolvida para obter uma visão geral precisa dos medicamentos antes da admissão, que envolve uma entrevista estruturada com os pacientes e tem demonstrado seu potencial para evitar erros de conciliação. O objetivo deste estudo foi adaptar interculturalmente o questionário SHIM para o espanhol. PACIENTES E MÉTODOS: Retrotradução com cegamento seguida de síntese e adaptação, com a participação de um painel de especialistas, para garantir a equivalência entre o questionário original e a versão em espanhol. Posteriormente, foi feito um teste-piloto da versão em espanhol por meio de entrevistas cognitivas em uma amostra de pacientes polimedicados sob acompanhamento do Departamento de Medicina Interna. RESULTADOS: Obteve-se a versão em espanhol do questionário SHIM (SHIM-e). Os escores de dificuldade atribuídos pelos tradutores envolvidos nas traduções direta e reversa foram baixos. Durante a fase de síntese e adaptação, três discrepâncias foram resolvidas, e o painel de especialistas decidiu incluir alguns termos comumente usados para entrevistas clínicas na versão espanhola do questionário. O teste-piloto, realizado em uma amostra de 10 pacientes polimedicados admitidos no Departamento de Cirurgia Geral e Digestiva, mostrou 100% de compreensibilidade para todos os itens, com exceção do número 13, que foi de 90%. CONCLUSÕES: Este trabalho apresenta a primeira adaptação intercultural para o espanhol do questionário SHIM. As traduções para direta e reversa apresentaram baixa dificuldade, e os resultados do teste-piloto mostraram um alto nível de compreensibilidade para a versão em espanhol desta ferramenta.
BACKGROUND AND OBJECTIVE: The "Structured HIstory of Medication use" (SHIM) questionnaire is a tool developed to obtain an accurate pre-admission overview of medications, involving a structured interview with patients, and has demonstrated its potential to prevent reconciliation errors. The objective of this study was to cross-culturally adapt the SHIM questionnaire to Spanish. PATIENTS AND METHODS: Forward and blind-back translations followed by a synthesis and adaptation, with the participation of an expert panel, to guarantee the equivalence between the original questionnaire and the Spanish version. Subsequently, pilot testing of the Spanish version was carried out through cognitive interviews in a sample of polymedicated patients under follow-up by the Department of Internal Medicine. RESULTS: The Spanish version of the SHIM questionnaire (SHIM-e) was obtained. Scores for difficulty assigned by translators involved in forward and back translations were low. During the synthesis and adaptation phase, three discrepancies were resolved, and the expert panel decided to include some terms commonly used for clinical interviews in the Spanish version of the questionnaire. The pilot testing, which was performed in a sample of 10 polymedicated patients admitted to the Department of General and Digestive Surgery, showed 100% comprehensibility for all items, except for number 13, which was 90%. CONCLUSIONS: This work presents the first cross-cultural adaptation to Spanish of the SHIM questionnaire. The forward and blind-back translations presented low difficulty and the results of the pilot testing showed a high level of comprehensibility for the Spanish version of this tool.
