Barreiras e fatores facilitadores nas transições de cuidado — comparando as perspectivas de profissionais da saúde hospitalares e comunitários
Quando um paciente deixa o hospital e volta para casa para receber atenção na comunidade, a qualidade do cuidado e a segurança do paciente são muitas vezes colocadas em risco. Este estudo procurou analisar o processo de alta hospitalar para identificar e comparar as barreiras e os fatores facilitadores no contexto do sistema onde ocorrem, a partir da perspectiva tanto dos profissionais da saúde hospitalares como comunitários. Os resultados foram derivados da análise de 348 relatos de incidentes, da observação de cinco reuniões de planejamento de alta com profissionais da saúde hospitalares, três grupos focais com profissionais hospitalares e seis grupos focais com profissionais comunitários. Identificamos cinco temas representativos das barreiras e quatro temas representativos dos fatores facilitadores deste processo, da perspectiva dos profissionais hospitalares e comunitários. Estes temas foram então discutidos no contexto dos subsistemas, hospitais ou ambientes de saúde comunitária onde ocorrem.
As a patient moves from hospital back home to receive community-based care, quality of care and patient safety are often put at risk. This study aimed to analyse the discharge process to identify and compare the barriers and facilitators within the context of the system in which they occur, from the perspectives of both hospital and community healthcare staff. The results were derived from the analysis of 348 incident reports, the observation of five discharge planning meetings with hospital staff, three focus groups with hospital staff, and six focus groups with community healthcare staff. Five themes representative of the barriers and four themes representative of the facilitators for this process were identified from both hospital and community healthcare staff's perspective. These were then discussed in the context of the subsystem, hospital or community healthcare setting, in which they occur.
